Overview Gonorrhea is a sexually transmitted infection (STI), also known as a sexually transmitted disease (STD). Gonorrhea is caused by a bacterium that infects both males and females. Gonorrhea most often affects the urethra, rectum or throat. In females, gonorrhea can also infect the cervix. Gonorrhea is most commonly spread during vaginal, oral or anal sex. But babies of infected mothers can be infected during childbirth. In babies, gonorrhea most commonly affects the eyes. Abstaining from sex, using a condom if you have sex and being in a mutually monogamous relationship are the best ways to prevent sexually transmitted infections.
Antiandrogen therapy: hormonal therapy with antiandrogenic medications such as spironolactone , flutamide , cyproterone acetate , ethinylestradiol , finasteride , dutasteride , and metformin have been found to be effective in clinical studies. [29] [30] [31] However, the quality of available evidence is low and does not presently allow for robust evidence-based recommendations. [29] [30] Intravenous or subcutaneous infusion of anti-inflammatory ( TNF inhibitors ; anti- TNF-alpha ) drugs such as infliximab , and etanercept . [32] This use of these drugs is not currently Food and Drug Administration (FDA) approved and is somewhat controversial, so may not be covered by insurance.
Overview Hidradenitis suppurativa (hi-drad-uh-NIE-tis sup-yoo-ruh-TIE-vuh), also known as acne inversa, is a condition that causes small, painful lumps to form under the skin. The lumps usually develop in areas where your skin rubs together, such as the armpits, groin, buttocks and breasts. The lumps heal slowly, recur, and can lead to tunnels under the skin and scarring. Hidradenitis suppurativa tends to start after puberty, usually before age 40. It can persist for many years and worsen over time. It can affect your daily life and emotional well-being.
Mosquito-borne Zika virus is suspected to be the cause of 2,400 possible cases of microcephaly and 29 infant deaths in Brazil in 2015 (of the 2400 or so notified cases in 2015, 2165 were under investigation in December 2015, 134 were confirmed and 102 were ruled out for microcephaly). [106] The Brazilian Health Ministry has reported at least 2,400 suspected cases of microcephaly in the country in 2015 as of 12 December, and 29 fatalities. [106] [107] [108] [109] Before the Zika outbreak, only an average of 150 to 200 cases per year were reported in Brazil. [110] In the state of Pernambuco the reported rates of microcephaly in 2015 are 77 times higher than in the previous 5 years. [110] A model using data from a Zika outbreak in French Polynesia estimated the risk of microcephaly in children born to mothers who acquired Zika virus in the first trimester to be 1%. [111] On 24 January 2016, the WHO warned that the virus is likely to spread to nearly all countries of the Americas, since its vector, the mosquito Aedes aegypti , is found in all countries in the region, except for Canada and continental Chile . [112] [113] The mosquito and dengue fever have been detected in Chile's Easter Island, some 3,500 km (2,200 mi) away from its closest point in mainland Chile, since 2002. [114] In February 2016, WHO declared the outbreak a Public Health Emergency of International Concern as evidence grew that Zika is a cause of birth defects and neurological problems. [18] [115] [116] [117] In April 2016, WHO stated there is a scientific consensus, based on preliminary evidence, that Zika is a cause of microcephaly in infants and Guillain–Barré syndrome in adults. [9] Studies of this and prior outbreaks have found Zika infection during pregnancy to be associated with early pregnancy loss and other pregnancy problems. [118] [119] Asia [ edit ] In 2016 imported or locally transmitted Zika was reported in all the countries of Asia except Brunei, Hong Kong, Myanmar and Nepal. [120] Serological surveys have indicated that Zika virus is endemic in most areas of Asia, though at a low level. [120] While there was a sharp rise in the number of cases of Zika detected in Singapore after the 2016 Summer Olympics in Brazil, genetic analysis revealed that the strains were more closely related to strains from Thailand than from those causing the epidemic in the Americas. [121] [122] [123] History [ edit ] Origin of the name [ edit ] It is named after the Zika Forest near Entebbe , Uganda , where the Zika virus was first identified. [124] Microcephaly and other infant disorders [ edit ] Zika virus was first identified in the late 1940s in Kampala, Uganda, Africa but was first confirmed in Brazil.
Zika virus disease is an emerging Aedes mosquito-born virus disease characterized by a clinical course that may be asymptomatic or mild with fever, conjunctivitis, muscle and joint pain, headache, exanthema, but may also be associated with severe neurological (meningitis, meningoencephalitis and myelitis) and auto-immune (Guillain-Barre syndrome) complications, as well as a potential increase of birth defects (microcephaly) if the infection occurs during pregnancy.
Thus, among Japanese with ataxia, SCA36 has a relatively low frequency, far below that for SCA6 (∼14%), SCA3 (∼11%), SCA31 (8%-17%), and DRPLA (∼5%), and also lower than other SCA types [Sakai et al 2010, Sugihara et al 2012] (see Hereditary Ataxia Overview).
Spinocerebellar ataxia type 36 (SCA36) is a condition characterized by progressive problems with movement that typically begin in mid-adulthood. People with this condition initially experience problems with coordination and balance (ataxia). Affected individuals often have exaggerated reflexes (hyperreflexia) and problems with speech (dysarthria). They also usually develop muscle twitches (fasciculations) of the tongue and over time, the muscles in the tongue waste away (atrophy). These tongue problems can cause difficulties swallowing liquids. As the condition progresses, individuals with SCA36 develop muscle atrophy in the legs, forearms, and hands.
A number sign (#) is used with this entry because spinocerebellar ataxia-36 (SCA36) is caused by heterozygous expansion of an intronic GGCCTG hexanucleotide repeat in the NOP56 gene (614154) on chromosome 20p13. Unaffected individuals carry 3 to 14 repeats, whereas affected individuals carry 650 to 2,500 repeats (Kobayashi et al., 2011 and Garcia-Murias et al., 2012). Description SCA36 is a slowly progressive neurodegenerative disorder characterized by adult-onset gait ataxia, eye movement abnormalities, tongue fasciculations, and variable upper motor neuron signs. An autosomal dominant cerebellar ataxia type 1 that characterized by gait and limb ataxia, lower limb spasticity, dysarthria, muscle fasiculations, tongue atrophy and hyperreflexia. Epidemiology Spinocerebellar ataxia type 36 (SCA36) prevalence is unknown. SCA36 has been described in more than 100 families in Asia (Japan and China) and Europe (France, Italy, Poland, and Spain-Galicia). In a US population, SCA36 accounts for 0.7% of disease in a large undiagnosed ataxia cohort. Clinical description SCA36 presents in adulthood with ataxic dysarthria, truncal ataxia, limb ataxia, general hyperreflexia and variably occurring lower limb spasticity.
Oligoclonal bands in NMO are rare and they tend to disappear after the attacks, while in MS they are nearly always present and persistent. [34] It is important to notice for differential diagnosis that, though uncommon, it is possible to have longitudinal lesions in MS. [35] Other problem for diagnosis is that AQP4 -ab in MOG -ab levels can be too low to be detected. Some additional biomarkers have been proposed. [36] [37] NMO differs from MS in that it usually has more severe sequelae after an acute episode than standard MS, MS infrequently presents as transverse myelitis , and oligoclonal bands in the CSF , as well as white matter lesions on brain MRI , are uncommon in NMO, but occur in over 90% of MS patients. [38] Recently, AQP4 has been found to distinguish standard MS from NMO, but as MS is a heterogeneous condition , [39] and some MS cases are reported to be Kir4.1 channelopathies [40] ( autoimmunity against the potassium channels ) it is still possible to consider NMO as part of the MS spectrum. ... Neuromyelitis optica spectrum disorders (NMOSD) affect the spinal cord and optic nerves (nerves that carry visual messages to and from the brain). Symptoms include pain, weakness, bowel and bladder problems, and temporary vision loss. NMOSD usually occurs in adulthood, but symptoms may start at any age. Some people have a single attack of symptoms lasting months, but in most people the symptoms come and go over time. People with NMOSD may develop permanent muscle weakness and vision loss.
Neuromyelitis optica is an autoimmune disorder that affects the nerves of the eyes and the central nervous system, which includes the brain and spinal cord. Autoimmune disorders occur when the immune system malfunctions and attacks the body's own tissues and organs. In neuromyelitis optica, the autoimmune attack causes inflammation of the nerves, and the resulting damage leads to the signs and symptoms of the condition. Neuromyelitis optica is characterized by optic neuritis, which is inflammation of the nerve that carries information from the eye to the brain (optic nerve ). Optic neuritis causes eye pain and vision loss, which can occur in one or both eyes.
Overview Neuromyelitis optica (NMO) is a central nervous system disorder that causes inflammation in nerves of the eye and the spinal cord. NMO is also called neuromyelitis optica spectrum disorder (NMOSD) and Devic disease. It occurs when the body's immune system reacts against its own cells. This happens mainly in the optic nerves that connect the retina of the eye with the brain and in the spinal cord. But it sometimes occurs in the brain. The disorder may appear after an infection.
Neuromyelitis optica (NMO) and NMO spectrum disorders are inflammatory demyelinating diseases of the central nervous system characterized mainly by attacks of uni- or bilateral optic neuritis (ON) and acute myelitis. Epidemiology NMO has a worldwide distribution and estimated prevalence of 1-2/100,000. Clinical description Patients present with acute, often severe, attacks of blindness and paraparesis or quadriparesis, accompanied by sensory and sphincter impairments. Most patients have relapsing attacks (separated by months or years with partial recovery), with usually sequential index episodes of ON and myelitis. A relapsing course is more frequent in women, and nearly 90% of patients are female (typically late middle-aged).
MRI is the diagnostic modality of choice for imaging prior to biopsy and pathologic diagnosis, with the primary role being the determination of anatomic boundaries. [ citation needed ] Staging [ edit ] The staging for epithelioid sarcoma takes into account size and location of the primary tumor, lymph node involvement, presence and location of metastasis, and histologic grade (a measure of disease aggressiveness) [26] Treatment [ edit ] Surgical resection of the tumor with wide margins remains the preferred method of treatment, [27] and has shown the most success against the disease. [27] [28] [29] Recently, limb-sparing surgery has been explored with moderate success. [30] In cases of advanced, recurrent, or metastasized disease, or if the tumor is inoperable, chemotherapy and radiation are the standard of care, [31] although the overall success rates with these remains low. [32] In January 2020, The U.S. Food and Drug Administration approved Tazverik (tazemetostat), a compound that blocks the EZH2 methyltransferase for the treatment epithelioid sarcoma in patients aged 16 years and older with either metastatic or locally advanced (unable to be completely removed surgically) disease. [33] Prognosis [ edit ] The 5-year survival rate for epithelioid sarcoma patients is 50-70%, and the 10-year survival rate is 42-55%.
Epithelioid sarcoma (ES) is a rare cancerous tumor that most often occurs in the soft tissue of the fingers, hands and forearms of young adults. It can also occur elsewhere in the body. ES usually begins as a painless, firm growth or bump that may be accompanied by an open wound ( ulceration ) in the skin covering the growth. This type of tumor often comes back after treatment or spreads to other parts the body (metastasis). The cause of epithelioid sarcoma is unknown. It is diagnosed by a clinical examination and by testing a small sample of the tumor (biopsy) in a laboratory. Epithelioid sarcoma is treated with surgery to remove all the cancer cells ( wide local excision ) and sometimes with radiation therapy.
Epithelioid sarcoma is a rare, soft tissue tumor characterized by high incidence of local recurrence, regional lymph node involvement and distant metastases. It commonly affects the soft tissue under the skin of a finger, hand, forearm, lower leg or foot, less often other areas of the body.
Serological studies suggest that H5N1 seroconversion in market poultry is low and that vaccination may have facilitated the selection of the Fujian-like sublineage.
Acute GvHD is staged as follows: overall grade (skin-liver-gut) with each organ staged individually from a low of 1 to a high of 4. Patients with grade IV GvHD usually have a poor prognosis.

Overview Shellfish allergy is an atypical response by the body's immune system to proteins in certain marine animals. Marine animals in the shellfish category include crustaceans and mollusks.
